Live Well

The following activities are designed to improve or maintain your physical health and wellbeing. Options include different physical activities, proper nutrition, and a health and fitness assessment. Each activity is worth 100 points; participants must earn 100 points from the Live Well category.

All benefit-eligible employees may complete the Be Well Rewards program. If you feel you need modifications to complete an activity because of your location, please email bewell@usu.edu and we will work with you to provide an appropriate modification or alternative

Attend an Employee Wellness Class

100 points

The Employee Wellness Program offers a number of different virtual and in-person fitness classes for all levels. From yoga and pilates to Cycling and weight training, there is a class for everyone. Virtual classes will be held via Zoom and recordings will be available for later viewing. Register and attend any class offered through the Employee Wellness Program, excluding the online arthritis class, and attend at least 75% of the class.

Complete One Free Personal Training Session

100 points

Whether you have just started contemplating your wellness journey or are ready to get started now, our Personal Trainers are here to help! Schedule a virtual or in-person one (1) hour training session that will include a brief health history assesment, address goals and concerns, and a personalized exercise program with the tools to continue and maintain on your own. Select 'Completing for Live Well requirement' when scheduling.

If needed, additional one (1) hour long sessions may be purchased.

Trek to Wellness

100 points

Hike, swim, cross-country ski or run 100 miles during the rewards period to earn 100 points! Miles can be tracked using the excel tracking sheet. Once your miles are complete, upload your tracking sheet via the submission page.

Documentation
Track your miles in the excel tracking sheet, and upload that when you've reached all 100 miles.

Create Your Own Activity Option

Don't see an activity that interests you? Maybe you are already doing something that could meet the requirements? New this year, you can create your own activity for the Live Well category! Activities need pre-approval from the Be Well program before submitting for documentation. Email bewell@usu.edu for approval of activity, determine necessary documentation requirements and any other questions.

Complete a Hearing Screening

100 points

Hearing loss can be disruptive to communication, affecting relationships, work and socializing. Hearing loss is also associated with mental illness and long-term cognitive effects. Employees have two free options to get their hearing tested through the Sorenson Center for Clinical Excellence. Please see a healthcare provider if you experience sudden change in hearing, ear pain and/or discharge. 

Logan Campus Employees: Contact the clinic at 435-797-1374 or sorensonhearingandbalance@usu.edu to schedule an appointment. Plan for one (1) hour to complete the screening. Upload the audiogram results letterhead with date of appointment and audiologist signature. Letterhead example

Complete a Fitness Assessment

100 points

Complete a Fitness Assessment through the USU Be Well Program. Fitness assessments consist of biometric screenings, a walking test as well as upper body, lower body and core strength measurements. Participants will receive a coupon for three free personal training sessions that must be used within a year of the date of assessment. Schedule a Fitness Assessment

Statewide Campuses: Employees can complete the at-home fitness assessment and do virtual personal training sessions. Email bewell@usu.edu for instructions to complete an at-home fitness assessment.

Attach your results page, or just screenshot the email showing your scheduled appointment for the assessment.

At-home Workouts from the Be Well Social Media Pages

We understand schedules can be complicated and lack of time may not allow you to exercise at a gym or on campus - so we are bringing the workouts to you! Complete a minimum of 15 of the at-home workouts found on our Facebook and Instagram pages and record the date and which workout you completed. Additional exercises, stretches and movements can be found on our YouTube page as well. Don't forget to 'Like', 'Follow' and 'Subscribe' for updates and information from the Be Well Program!

Record which workout you completed and the date and submit it to your dashboard.

Bike to Wellness

100 points

Bike to work, around your community, for exercise, or for fun! If you can pedal your way to 160 miles during the rewards period, you can earn 100 points! Track your miles using any biking app or fill out the excel tracking sheet and upload it via the submission page.

Documentation
Track your miles in the excel tracking sheet, and upload that when you've reached all 160 miles.

Get a Flu Shot

100 points

Get your annual flu shot either at your local pharmacy, primary care physicians office, or at our annual Flu Shot Clinic on Logan campus. For more information regarding the Influenza vaccine or to register for a Logan Campus clinic, visit flushots.usu.edu.

Documentation
Make sure to select the 'Flu Shot' activity in ServiceNow so we will be notified but please do not upload any personal health information.

Visit a Gym or Recreation Facility

100 points

*All Campuses

Visit a local gym, community center, recreation center or a USU campus exercise facility twenty-four (24) times between February 1, 2024 and May 31, 2024. For non-USU facilities, please submit an attendance log from your gym. Most facilities will gladly print this out at the front desk.

Documentation
Logan campus facilities: we will get your entries from the computer system to check for attendance.
Statewide campus or non-USU facilities: Get a printout showing your attendance (most front desks will give this to you upon request), or track it on your own in an excel sheet (please include dates).

Race Registration

100 points

Register and complete any race, fun run, or walking events totaling 15k or longer during the reward session and recieve 100 points. You may combine multiple events to reach the 15K total distance. Scan your registrations or take a picture of yourself during the event! Upload you images via our submission page.

Get a screenshot of an email showing the registration receipt, scan other payment receipts or your race bib, or take a few pictures of you at the race!
