Attend Employee Wellness Class

The Employee Wellness Center offers a number of different fitness classes for all levels. From yoga and pilates to Cycling and weightliting, there is a class for everyone. Register and attend any class offered through the Employee Wellness Center. Classes can be found HERE.

For documentation: Either scan your registration receipt, payment receipt, or snap a picture of you at class!

Nutrition Consultation with a follow up appointment

Meet with our Be Well health coach Sunny Stewart for a 30-45 minute nutrition assessment. Sunny will help you review your current diet and nutritional habits and set goals for the future. We require one follow-up visit with Sunny to allow her to make any additional changes or set additional goals. You can reach Sunny at sunny.stewart@usu.edu to setup an appointment.

For documentation: You can screenshot any correspondence email you have with the nutritionist, or anything showing some of the things you worked on with her.

Trek to Wellness

Hike, swim, or run 100 miles during the rewards period to earn 100 points! Miles can be tracked using the excel tracking sheet HERE. Once your miles are complete simply upload your tracking sheet via the submission page.

For documentation: Track your miles in the excel tracking sheet, and upload that when you've reached all 100 miles.

Complete a Fitness Assessment and enjoy a free personal training session.

Complete a Fitness Assessment through the USU Be Well Program. Fitness assessments consist of a walking test as well as upper body and core strength measurements. Participants receive one free personal training sessions for completing a fitness assessment.
*Employees at regional campuses may complete the at home fitness assessment. Email BeWell@usu.edu for instructions to complete an at home fitness assessment.

For documentation:  Attach your results page, or just screenshot the email showing your scheduled appointment for the assessment.

Bike to Wellness

Do you currently bike to work, for exercise, or for fun? If you can pedal your way to 160 miles during the rewards period you can earn 100 points! Just track your miles using any biking app or fill out the excel tracking sheet HERE and upload it via the submission page.

For documentation: Track your miles in the excel tracking sheet, and upload that when you've reached all 160 miles.

Age Appropriate Health Screenings

Complete your age appropriate health screening with your provider. For example mammogram, endoscopy, PSA, dental exam, or annual physical. You may count your annual physical if it was completed after January 1, 2017. Please DO NOT indicate what screening/procedures were done.

For documentation:  Because of HIPAA laws, we cannot legally require proof of doctor's visits, but if you are still inclined to provide some documentation, just make sure to exclude any personal or health related information.

Gym Membership
* All Campuses

Attend the Employee Wellness Center, Nelson Fieldhouse or any local gym, community center or rec. center 24 times between February 1, 2017 and May 31, 2017. For non-USU facilities please submit an attendance log from your gym. Most gyms will gladly print this out at the front desk.

For documentation:  For those attending on-campus gyms, we can get your entries from the computer system to check for attendance.  For all outside gyms, please get a printout showing your attendance (most front desks will give this to you upon request), or track it on your own in an excel sheet (please include dates).

Race Registration

Register and complete any race, fun run, or walking events totaling 15k or longer and recieve 100 points. You may combine multiple events to reach the 15K total distance.
Scan your registrations or better yet snap a picture of yourself during the event! Upload you images via our submission page.

For documentation:  Get a screenshot of an email showing the registration receipt, scan other payment receipts or your race bib, or take a few pictures of you at the race!
