**Due to on going research into the effectiveness of the Flumist nasal spray vaccine, it has been determined by the CDC that there will be no Flumist available for the 2017-2018 flu season. In accordance with the CDC decision all participants will receive the flu vaccine via injection.

Note: For the 2017-2018 season, CDC recommends use of the flu shot (inactivated influenza vaccine or IIV) and the recombinant influenza vaccine (RIV). The nasal spray flu vaccine (live attenuated influenza vaccine or LAIV) should not be used during 2017-2018. The 2017-2018 influenza vaccination recommendations are now available.
- You must register for yourself and ALL dependents receiving a flu shot at the Clinic
- Bring your Blue Cross Blue Shield (BCBS) insurance card to the Flu Shot Clinic
- Please complete this form prior to the Flu Shot Clinic and bring it with you to the Clinic.
If you are unable to make it to the annual on campus flu shot clinic you may recieve free flu shots for you and your dependents at any pharmacy within the Regence BlueCross BlueShield of Utah network.
